In short
- This form is for exercising your rights under Article 11 of Law No. 6698.
- Please do not attach a health report, measurement result or copy of your ID to your request.
- Your request is answered as soon as possible according to its nature and within 30 days at the latest.
- Applying is free. A fee may be charged if the reply exceeds 10 pages or is given on a medium such as a CD or memory stick.
This form is for exercising your rights under Article 11 of the Personal Data Protection Law No. 6698 (the "Law") with Murat Erdoğan ("ME Consultancy"). It has been prepared under the Communiqué on the Procedures and Principles of Application to the Data Controller.

How your request is answered
- Your request is answered as soon as possible according to its nature and within 30 days at the latest.
- Applying is free. If the reply exceeds 10 pages, 1 Turkish Lira may be charged for each page above that, and if the reply is given on a medium such as a CD or memory stick, the cost of the medium may be charged. If the request arises from our error, the fee is refunded.
- We may ask for additional information to verify your identity.
- If your request is refused or you find the reply inadequate, you may complain to the Personal Data Protection Board within 30 days of learning the reply and in any case within 60 days of your application.
- This form and the information in your request are processed only to conclude and evidence your request and are kept for 3 years under Law art. 5/2(ç) and art. 5/2(e).
